I hereby authorize my employer, in terms of the Labour Act [Chapter 28:01] 54 Section as may be amended from time to time to deduct from my salary / wages the appropriate amounts set out below and transfer the money to the Union ZEHSSCWU as per its written instruction as my membership dues on a monthly basis and this authorization shall remain in force until instructed by the Union ZEHSSCWU in writing

1. Joining Fee to be paid once during the first month on admission.
2. Monthly subscriptions to be deducted and remitted to the Union ZEHSSCWU not later than the 7th day of the month following the deductions.
3. Any levies which the Union's National Council may decided from time to time.
